Please write a short autobiographical sketch that describes your spiritual journey up to this point, including your family background and the significant happenings in your spiritual journey and any ministries where you participate. Also indicate your reason for wanting to participate in the program.

You may write out your personal essayhere or upload a document to attach.

Personal References
Please list the names of two (2) people who give a recommendation for you to the program. (A pastor, spiritual director or mentor would be appreciated for one of the references). When you have completed this application there will be a separate form to send to your references to have them fill out and submit back to us.

Please submit your $50 non-refundable application fee to complete your application. If you are selected for the program your application fee will be put toward your program fee.
